Person Centered Planning
Bright Path Independent Facilitators empower individuals by simplifying the self-determination journey, offering person-centered planning, tailored support, and advocacy to help clients achieve autonomy and independence.
We provide guidance through person-centered planning, helping clients define their goals, allocate budgets, and access resources tailored to their Individual Program Plan (IPP). '
By advocating for clients and fostering empowerment, our facilitators ensure that individuals can make informed decisions, take control of their lives, and achieve greater independence.
Transition To SDP
Bright Path focuses on empowering individuals to maintain their Self-Determination plan by offering personalized support tailored to their unique needs and goals.
We ensure that the plan remains aligned with the individual's preferences and aspirations, regularly reviewing and adjusting it as circumstances change.
Bright Path provides resources, tools, and guidance to help individuals navigate challenges and make informed decisions, fostering a sense of autonomy and confidence.
